REQUEST FOR TRANSPORTATION TO AND FROM CHILD CARE

PLEASE NOTE: Child Care arrangements are not carried over from one school year to the next.
A child care transportation request must be filled out every year.

Child Care Transportation Guidelines:

Child care provider must be located within the same school zone of attendance as the
student's residence. Example: If your child attends Chorley Elementary school, the child
care provider must be located within the Chorley Elementary school zone.

Requests will be denied if the child care provider is located within a walk zone (less than 1 mile)
of the school of attendance.

Child care provider must be located at a residential address

If your request is for both AM & PM, student must go to the same child care provider
both AM & PM

Student must be picked up from or go to the child care provider Monday thru Friday

Requests will be denied if the school bus you are requesting is full or if there has been a
discipline problem.

If the bus becomes overcrowded during the school year because new students move into
the district, or if there is a discipline problem, child care transportation can be rescinded.

All Pre-K & Kindergarten students must be met at the bus stop daily by a responsible adult
